RESUMO

The purpose of this article is to report on an innovative new model of care and the effects this model pilot program had on patient satisfaction, staff satisfaction, physician satisfaction, patient quality metrics, and financial metrics. The Virtually Integrated Care team is a model of care that leverages technology to bring an experienced expert nurse into the patients' room virtually. The advanced technology allows the virtual nurse to direct and monitor patient care, interacting with the patient through 6 core roles: patient education, staff mentoring/education, real-time quality/patient safety surveillance, physician rounding, admission activities, and discharge activities.

RESUMO

Access to essential health-promoting nursing care is increasingly facilitated through the expanding scope and capabilities provided through the utilization of telehealth technologies. Telehealth technologies facilitate the nursing process with clients at a distance in multiple ways, such as video conferencing, video conferencing with peripheral diagnostic tools, and store and forward modalities. Nursing care needs to remain the focus of care, but technology is a useful ancillary tool to help clients achieve health outcomes. The authors of this article discuss nursing practice and the use of telehealth technology and how King's concepts and theory of goal attainment can be used as a guiding framework.

Health technology has been proposed as a route to financial savings and improved patient safety for many years within the NHS. Nurses have a key role to play in transforming care through such technology but, despite high-level endorsement, implementation of health technology has been uneven across NHS community services. This article looks at three promising applications of health technology in community nursing: mobile access to digital care records; digital imaging; and remote face-to-face consultations. Current evidence for these technologies gives some indication of what is required before health technologies can benefit patients. Rapidly changing health technologies make it difficult for community services to make fully informed decisions when implementing them. There are challenges in predicting the full financial and efficiency impacts, in making robust estimates of costs and workload implications and in anticipating the effects on patient care and staff experience. Despite these problems, there is mounting evidence of the benefits of technological innovations available to community nurses and their patients.

AIMS AND BACKGROUND: Telenursing is the use of technological resources and communication systems to encourage the development of nursing. Its efficiency has been demonstrated to help countries overcome barriers to healthcare. This study investigates the current telenursing strategies utilized in nursing practice, as found in the literature. METHOD: Integrative literature review of the application of telenursing, using the descriptors: telenursing, nursing care and communication means, in Portuguese, English and Spanish, between 2003 and 2013. RESULTS: Telenursing is found particularly in care through telephone use for health services and orientations. The country with the largest number of research publications was the United States with 14 (37.8%), followed by Canada and the United Kingdom. FINAL CONSIDERATIONS: It could be verified that telenursing is growing, in view of its presence in different countries, with strong evidence and benefits of its use. It proves to be an efficient tool to help countries overcome geographical barriers and provide health care information to the population.

La teleostomía y la telecura son consultas virtuales que permiten al paciente recibir cuidados especializados en el propio domicilio. Con esto vamos a conseguir que los pacientes tengan que desplazarse con menor frecuencia al centro de referencia, mejorando así el confort y tranquilidad que les ofrece su propio hogar. Dicha consulta facilita la tarea diaria de enfermería de Atención Primaria contando con el apoyo de profesionales expertos, que pueden orientar, ayudar y guiar en la realización de los cuidados específicos relacionados con las heridas crónicas y/u ostomías. Esto le permitirá al profesional ser más resolutivo, a la vez que se reducirán los ingresos hospitalarios y consecuentemente el gasto sanitario
